Dear Individual Income Tax Section of NC-DOR:

You sent me your Notice of Collection forms for tax years 1999; 2000; 2001; 2002;
2003; 2004; 2005, dated November 12, 2008.
I request a conference or a meeting with DOR for these years so we may discuss 
collection alternatives.
I request for you to waive penalties. Please send me the necessary forms and 
information so I can make proper requests and reach a settlement in this matter.
I am unable to pay the amount you say I owe. I have a dependent wife and other 
dependents and many business expenses. I do not have enough money to pay my bills.
Please schedule a time so we may meet to discuss collection alternatives.
