I received your letter dated 3/1/2007 (copy attached). I hereby request a written statement of the information upon which the income tax assessments against me for the Tax year 2004. Your said letter stated my assessment was based on information you obtained from Employment Security Commission.

Then you stated your information was obtained from The Internal Revenue Service. Which one is it? I deny both figures. Apparently some Illegal immigrant has been using my Social Security Number.

The proposed assessment is incorrect. I have factual evidence to support that the assessment is in error. I hereby request a hearing and also have much information for you to consider for the tax year in question. I had un-reimbursed expenses such as: medical/dental, tax returns expenses, charitable, and church donations. I am also a disabled veteran with greater than 50% hearing loss in the right ear. Please let me know the best way to submit my books and/or records for your review.
