This is a request under The Right to Financial Privacy Act, 12 USC §3401, the Privacy Act of 1974, 5 USC §552a, and The Special Procedures for Third-Party Summons Act, 26 USC §7609, part of The Tax Reform Act of 1976.
    I request that you, your agents and employees not reveal, divulge, or disclose to any person, organization or agency, private or governmental, under any excuse or pretext whatsoever, any information or documentation concerning me or of any organization connected with me, without my express written permission.
     Further, you are to immediately advise me of any request, whether by letter or telephone. Please return to me the original and all copies of any credit application, loan papers, signature cards or other documents concerning me that you have retained in your files.
     You are aware, of course, that revealing my records without my permission, except under a legitimate court order, is a violation of my privacy.
