This is an official request for information concerning income paid to Ms. Kathy Patriot (SSN 123-45-6789) for the taxable year 2007. Ms. Patriot claims that the information in our records is incorrect regarding her 1099s for that year.

We are therefore requesting that you provide us with documentation to corroborate the amount of income that was earned by Ms. Kathy Patriot and reported to the Internal Revenue Service for the taxable year 2007.
